Online Training Request Form

The Online Training Approval Form is for organizations and instructors of computer-based training courses that are not live and interactive. These trainings are pre-recorded and may require an online examination upon completion. This Form is not intended for students, i.e. persons taking the training.

  • PresenterAlternate Presenter 
    Select your presenter(s) from the dropdown list. The dropdown list includes the names of presenters who have already submitted their resumes to the Board as part of a prior Training Approval Request. If you do not find your presenter, select 'Other', then fill in your presenter's name in the 'Alternate Presenter' column. Max of 3. Please list additional presenters in body of material uploaded later in this application. Note: The NCSAPPB does not have the statutory authority to approve presenters; only training courses, conferences, and workshops are considered approved by the NCSAPPB

    Thank you for completing a NCSAPPB training approval request. You will receive an email confirming a successful application submission. You will then receive an approval email with a link to make payment. If you have not received the email with the link to make payment within a week, please check your spam / junk mail folder. Once payment has been made, your request will then be reviewed by staff member Donna Strickland and you will receive an email with the approval information.
